    How To Obtain Certified Copies for Birth, Marriage or Death

    Registrar Records -

    Applying for a Marriage/Civil Union License in NJ or Requesting a Certified Copy of a Vital Record

    The Medford Township Registrar maintains records of all births, marriages, civil unions and deaths occurring in Medford Township.  Certified copies with a raised seal, are only available to the individual on the requested record or the individual's parents, current spouse, child, grandchild, sibling, legal guardian or legal representative. 

    • Certified copies of birth, marriage, civil union, domestic partnership or death records are $25 for the first copy of a vital record; $5 for each additional copy of the same record ordered at the same time.  Payments are to be made via cash, check or money order.  We do not accept credit cards or debit cards.  

    For two persons to establish a Marriage/Civil Union in this State, it shall be necessary that they satisfy all of the following criteria:

    • Not be a party to another civil union, domestic partnership or marriage in this State or recognized by this State; and
    • Be of the same sex for civil union; marriage can be same or opposite sex couples
    • Be at least 18 years of age, except that applicants under the age of 18 may enter into a Marriage/Civil Union with parental consent.  Applicants under the age of 16 must obtain parental consent and have the consent approved in writing by any judge of the Superior Court, Chancery Division, Family part.
